What ages do you treat?

We specialize in adult medicine, treating patients 18 years and older. Our providers have additional training in internal and geriatric medicine for aging adults.

How do I get prescription refills?

You can request a refill by contacting your pharmacy or logging into our patient portal. If needed, the pharmacy will send us the request directly. For controlled substances or special cases, please call our office at (248) 912-1370.

What should I bring to my first appointment?

Please bring a valid photo ID, insurance card, a list of medications, and any recent medical records. Arrive 15 minutes early to complete paperwork if you haven’t done so online.

Can I see the same provider each time?

Absolutely. Continuity of care is important to us. We strive to schedule patients with their preferred provider whenever possible.

Do you provide telehealth or virtual visits?

Yes. Through our sister practice, Avalon Physician Services, we provide home-based and facility-based care for eligible patients in assisted living, memory care, and other residential settings. This allows you to receive the same level of expert care outside the traditional office, when needed.

What happens if I can no longer come to the office for care?

If your health needs change, Avalon Primary Care works closely with our sister practice, Avalon Physician Services—the highest-rated housecall group in Michigan. We can coordinate a smooth transition to in-home or facility-based care, ensuring you continue receiving high-quality, personalized support.
